Your First Appointment

If you are a first-time patient, please plan on arriving early to complete the patient registration forms. Please bring the following items to the appointment:

• Insurance card(s)
• Driver’s License
• Referral forms (if required) and required co-payment or deductible
• List of all medications you are currently taking (prescribed/over the counter)
• Pertinent medical and surgical history information
